*Deterioration of personal hygiene

*Falls or change in ambulatory function

*Wandering from home, getting lost, forgetful, or losing items

*Decrease in vision and hearing

*Frequent emergency room visits

*Difficulty with bathing, dressing, preparing meals, using the bathroom, or other activities of daily living activities

*Unable to self-administer medications or not take medication as prescribed

*Unsanitary living conditions or hoarding

*Increased confusion or heightened confusion in the evening, resulting in agitation or combativeness

*Diagnosis of dehydration, malnutrition, or frequent urinary tract infection

*Poor safety awareness/unsafe behaviors such as leaving the stove on, unlocked door, etc

*Unpaid bills, collections notices unexplained
